Terren S. Peizer
Founder, Chairman of the Board, and CEO

Mr. Peizer is an entrepreneur and an investor with a particular interest in healthcare, having founded and successfully commercialized several healthcare companies. Mr. Peizer is the founder of Ontrak, Inc., a publicly traded leader in behavioral and mental health management services. He has also served as the Company’s Chairman of the Board of Directors and CEO since the Company’s inception in 2004. Mr. Peizer is also the Founder, Chairman and CEO of NeurMedix, Inc., a biotechnology company with a focus on inflammatory, neurological and neuro-degenerative diseases. NeurMedix’s lead compound NE3107 has shown efficacy in 11 pre-clinical and 6 human studies, resulting in publications in peer-reviewed journals. Mr. Peizer is also the Chairman, CEO and majority shareholder of publicly traded BioVie, Inc. which is the industry leader in the development of two orphan drug candidates for the treatment of the rare liver diseases Ascites and HRS. Mr. Peizer is Chairman of Acuitas Group Holdings, LLC, his personal holding company, and the owner of all of his portfolio company interests, including Ontrak, Inc. Through Acuitas, Mr. Peizer also owns Acuitas Capital Group, LLC, an industry leader in investing in publicly traded micro and small capitalization equities, having invested over $1.5 billion directly into portfolio companies. Mr. Peizer has held senior executive positions with several other publicly-traded growth companies, including Chairman of internationally renowned Cray, Inc.. Mr. Peizer has a background in finance and capital markets having previously served in senior executive positions with the investment banking firms Goldman Sachs, First Boston, and Drexel Burnham Lambert. He received his B.S.E. in Finance from The Wharton School, University of Pennsylvania.


Richard A. Berman
Director

At the University of South Florida, Richard A. Berman serves as interim dean of the Patel College of Global Sustainability, visiting social entrepreneurship professor in the Muma College of Business, and professor in the Institute for Innovation and Discovery. A recognized global leader, Mr. Berman has held positions in health care, education, politics, and management, including working with several foreign governments, the United Nations, the U.S. Department of Health and Human Services, and the U.S. Food and Drug Administration.Mr. Berman has also served as a cabinet level official for the state of New York and has worked with McKinsey & Company, NYU Medical Center, Westchester Medical, Korn-Ferry International, Howe-Lewis International, and numerous startup companies. From 1995 to 2009, he served as president of Manhattanville College, where he is credited with the turnaround of the college, doubling its enrollment during his tenure. Mr. Berman serves on the board of several organizations and is an elected member of the National Academy of Medicine of the National Academy of Sciences. Mr. Berman received his BBA, MBA, and MPH from the University of Michigan and holds honorary doctorates from Manhattanville College and New York Medical College.


Gus Giraldo
Director

Gus Giraldo serves as the President of Behavioral and Specialty Health at Magellan Healthcare, where he is accountable for the strategic development and profitable growth of the company’s health plan, employer, provider segments, state carve-out behavioral health and federal markets. With more than 20 years of leadership experience in the insurance industry, Mr. Giraldo has a strong track record of initiating, leading and growing domestic and global healthcare businesses. He is a managed care expert who brings a deep understanding of the dynamics of varied healthcare systems around the world, consumer marketing, product innovation and analytics. Prior to joining Magellan Healthcare, Mr. Giraldo served as Chief Operating Officer for Chubb’s Latin American life, accident and health business. Previously, he was at Cigna for 18 years, where he served in various leadership roles domestically and abroad. A Fellow of the Society of Actuaries, Mr. Giraldo earned his bachelor’s degree in mathematics from The Florida State University and his master’s degree in business administration from the University of Pennsylvania’s Wharton School of Business.

Rob Rebak
Director

Rob Rebak currently serves as CEO of Forefront Telecare, a behavioral telehealth company serving seniors in rural health systems in 20 U.S. states, where he is responsible for company vision, strategy, growth, culture, and overall operating performance. A 30-year healthcare industry veteran, Mr. Rebak was previously President and CEO of AbleTo, a behavioral telehealth company serving adult health plan members nationwide, and Chairman and CEO of QualityHealth, a digital patient acquisition and engagement platform. Earlier in his career, Mr. Rebak held commercial and leadership roles at Pfizer, Merck, and Bristol-Myers Squibb. In addition to serving on the Ontrak Board of Directors, he is a director for the Quell Foundation. Mr. Rebak holds an MBA in Finance and Marketing from the University of Chicago Booth School of Business and a BA in History and Economics from Vanderbilt University.

Diane Seloff
Director

Since 2013, Diane Seloff has served as COO of Aspire Health, a provider of home-based palliative care that was acquired by Anthem in 2018. Before joining Aspire Health, Ms. Seloff held senior executive roles in operations and development at other healthcare services and technology companies, including Vanderbilt University Medical Center, and served as a management consultant at Arthur Andersen & Company and KPMG Peat Marwick. Diane has a BS degree in Industrial and Operations Engineering from the University of Michigan and an MBA from Columbia University Business School.

Michael Sherman
Director

Michael Sherman recently retired as managing director in the investment banking division at Barclays PLC. Prior to Barclays, Mr. Sherman was managing director in investment banking at Lehman Brothers, Inc. Prior to Lehman Brothers, Mr. Sherman was an investment banker at Salomon Brothers inc. Mr. Sherman began his career as a securities lawyer with Cleary, Gottlieb, Steen & Hamilton in New York and Hong Kong. Mr. Sherman also serves on the Board of Directors of BioVie, Inc. Mr. Sherman earned his BA from the University of Pennsylvania, magna cum laude and law degree from Harvard Law School, cum laude.



Edward Zecchini
Director

Mr. Zecchini serves as the Chief Information Officer for Remedy Partners, Inc., where he is responsible for building the technology and infrastructure necessary to support the implementation of bundled payment programs and other episode financing business lines. Mr. Zecchini previously served as executive vice president and chief technology officer at Sandata Technologies from May 2010 to March 2014, president and chief executive officer of IT Analytics from March 2008 to April 2010, executive vice president of operations and chief information officer of Touchstone Healthcare Partnership from May 2007 to February 2008 and senior vice president and chief information officer of HealthMarkets from October 2004 to April 2007. Earlier in his career, he held senior-level positions at Thomson Healthcare and SportsTicker. Mr. Zecchini holds a Bachelor of Arts degree in Computer Science from the State University of New York at Oswego. He is also a Director of Cryoport Inc. (NASDAQ: symbol “CYRX.”).
